Licensed Practical Nurse (LPN)
Job Summary:
The Licensed Practical Nurse (LPN) is a front-line position being a representative for PJS Care Services in providing services carried out by the company. The LPN's duties are not limited to those listed as the position has a wide scope of duties at times dependent on the Client yet always under the auspices of the company.
The LPN will typically work unsupervised at Client Residences mostly independent or in a position supervising a Nursing Assistant and/or Aide. They must demonstrate the ability to balance good judgement and initiative along with implementation of the policies and procedures of the Agency.
The LPN will further be responsible for carrying out Client’s Plan of Care that will have been developed and signed off by a physician and administered under the supervision of an RN. It is the responsibility of an LPN, occasionally under the supervision of an RN Nurse Supervisor, to perform routine treatments and services in the delivery of care to Clients.
An LPN will serve as a staff nurse and is responsible for the daily work assignments of Nursing Assistants and Aides and will participate in their on the job training that will extend to providing information used in evaluating their performance. The LPN may also be required to provide Clients and their family members with medical education, consolation and listen to their concerns as well as frequently communicate with them.
The LPN should enthusiastically involve the Client in the care program and help provide the Client with the basic standards of healthy living which, among other things, includes helping the Client engage in physical activity, mental stimulation activities, relaxation activities and the provision of adequate nutrition.
Desired Qualifications:

Current RN license in the State of Illinois

Current certificate in standard first aid and CPR

Ability to Supervise (instruct, assign work, etc.) Nursing Assistants

Valid driver’s license and reliable transportation

Ability to work independently

Ability to work under the supervision of an RN Nurse Supervisor

Excellent communication and listening skills

Ability to work in a stressful environment

Must be physically and mentally fit to handle the demands of caregiving

Duties and Responsibilities:
The following list is a guideline of possible responsibilities and duties to be performed independently or as a group of duties in the course of working with the Client and in accordance with the Client’s Plan of Treatment. The LPN’s duties are not limited to those listed and are in accordance with the Illinois Nurse Practice Act:

Prepares and administers medications and treatments with knowledge of purpose and possible adverse effects and as prescribed by the Client's physician or primary health care provider.

Implements aspects of the Client’s Plan of Treatment (signed off by a Physician) and works towards maintenance of the Client’s health and well-being under the delegation of an RN Nurse Supervisor.

Completes all documentation for observation, planning, and evaluation of care provided to clients including those related to orders by physicians.

Develop and remain familiar with clinical notes for the Client's Clinical Record.

As applicable, report changes in the Client's condition to an RN Nurse Supervisor and document such changes in the Client's Clinical Record.

Functions in a monitoring capacity as assigned for completion of Client care and Nurse Assistant and Aide guidance.

Acts quickly and with medical knowledge in emergency situations.

Maintains open lines of communication between nursing shifts, the RN Nurse Supervisor, care team, physicians, and the Client's family and friends.

Is aware of safety issues, performs responsibilities in a safe manner and exhibits strong safety practices.

Works as part of a team in the implementation of the Client's Plan of Treatment.

Provides input to the Agency's governing body for the development and improvement of policies and procedures to support patient safety and the provision of quality care.
General Notes:
General Notes: The LPN will be required to maintain a safe working environment for themselves and the Clients. There may be emergency situations where the LPN will have to react quickly with sound judgement. The scope of duties described in this document may not include all duties to be performed by the LPN and is subject to change.
Place of Employment:
Client homes and work sites outside of the client home as required.
Work Hours:
This position may require various shifts at various times (including nights, weekends, and sleep over) typically in a single work site.
